The Celiac serology cascade follows a reflexive testing algorithm in which serum IgA levels will be initially measured. If the serum IgA level is <7.0 mg/dL, an anti-tissue transglutaminase (tTG) IgG assay will be performed. Otherwise, an anti-tissue transglutaminase (tTG) IgA assay will be performed. A positive result with any of the screening assays (anti-tTG IgA, anti-tTG IgG, anti-DGP IgA or anti-DGP IgG) supports the diagnosis of Celiac disease.

Specimen: Blood

Container: Serum Separator or Plain Red Top Tube

Volume: 6 mL; Minimum 4 mL

Pediatric Collection:

Two Gold or Red microtainers filled to 600 uL.

Storage Instructions:

Specimen may be held overnight at Room Temperature. Refrigerate for storage beyond 12 hours. Serum may be stored for up to 7 days at 2-8C. For longer storage, serum must be frozen at -20 or colder.

Turnaround Time (TAT):

Same day as performed

Test Availability:

IgA performed 24 hours a day / 7 days a week.  TTG and Gliadin testing performed on Monday – Friday.
